There is a big chance with this pack of forwards that Ansarifard will lose his TM place for the world cup, if everything continues the way it does.

SUBS:
1. Ansarifard => Rezaei
2. Kanaani => Pouraliganji
3. Jahanbakhsh => Dejagah (move Ghoddos into the middle)
It is vital that Torabi and Ghoddos play the full match. Saman needs to be tested as a winger, and as a CAM. And Torabi needs some time to show himself, Jahanbakhsh has been struggling, and Taremi and Ansarifard play a much different style on the wing, and I find Amiri to be a very average player who just has stamina, therefore the key people being tested is Ghoddos, Torabi, and Mazaheri, and also Ebrahimi to see how does as the sole CDM (something Ezatolahi can't do).

I think this is the most ideal lineup if we want to go into the World Cup with counterattacking and defensive style with speed on the flanks. I would use a 5-2-1-2 formation to see how that works. For Dutch NT in 2014, it worked fantastically.
We will have an incredible tough defense with three rocks in the back and Hosseini functioning as leader of the defensive line.
I seriously think that Jahanbakhsh might be better doing his defensive work rather than playing as a forward. He is a hard working player who can hold the ball. Maybe we should try him out as a right-back. In the World Cup 2014, Dutch veteran Dirk Kuyt was also used by Van Gaal as an upcoming back on the flank.
Haghighi
Mohammadi - Pouraliganji - Hosseini (C) - Cheshmi - Jahanbakhsh
Hajsafi - Ezatolahi
Dejagah
Ghoddos - Azmoun

Building on your formation (not including the Jahanbakhsh part because that may be too many changes, but just to show my point adapting your ideas and lineup)

What is this soft spot CQ has for Ansarifard? What the hell has he done for us ?!I can't think of one important goal he scored for TM in his whole career. He has had countless caps and has done nothing, yet he still gets invited over and over and over again ?! No one else is curious about this?? we can't even say hes 'young' anymore he is reaching peak football age pretty soon.You just got Caged
