By rehearsed do you mean if Van Persie practiced to score headers like that from crosses? If so then yes. I think he has practiced headers like that.

My question is, did the Dutch team plan a goal like that ahead of the game, or did van Persie instantaneously/suddenly decide to take the diving header? (Most players would choose to hit that cross with their foot stretched, for extra power.)

Yes, it was rehearsed so Van Persie could go down in history as the flying Dutchman. They practised it everyday for 3 months before the WC started. KEEP YOUR EYES OPEN PEOPLE! THERE'S A LOT THEY'RE TRYING TO HIDE FROM US.
